New Patients
We recommend completing the new patient form below and bring them with you to your scheduled appointment. If you are unable to complete the forms ahead of time, we ask that you arrive at your scheduled appointment 15- minutes ahead to complete the forms. Please remember to bring your insurance card, photo i.d, and medication bottles or lists with each and every visit.
Bill Payment
Woodbridge Medical Group accepts major credit cards as well as checks and cash. To pay your bill simply stop in the office during our office hours or call the office directly at (732)-636-5252 during office hours as well as Wednesdays from 8:30am-3:00pm. Our receptions will be happy to assist you.
